POLICY ON INCIDENT, ACCIDENT AND
HAZARD INVESTIGATION
Adopted by Parish Council 22 August 2004
1. Purpose
To document the procedures to follow for incident,
accident and hazard investigation.
2. Scope
This procedure applies to all accidents, injuries or
incidents with a potential for recurrence, and to all
hazards.
3. Procedures – Incidents and Accidents
3.1 All incidents and accidents will be reported to an
Executive member as soon as possible after the accident.
3.2 All incidents and accidents will be reported in the
Incidents and Hazards Book, kept in the Foyer of the
Church.
3.3 All incidents and accidents will be investigated by a
member of the Executive.
3.4 The incident and accident investigation will follow
the sequence of events from the incident back to the point
when the situation deviated from normal. Factors
investigated will include people, equipment and
environment and the interaction between them. The
controllable factors will be identified and a priority for
action on these factors taken to prevent a recurrence.
3.5 The Executive will complete any action resulting from
the investigation as soon as possible after the incident.
The Executive will keep Parish Council briefed.
3.6 Where the action plan involves expenditure which
exceeds available Parish funds, Parish Council will pursue
sources of funding from the Diocese.
3.7 Where an action is identified which cannot be met the
Executives will communicate this to all people involved.
4 Procedures – Hazards
4.1 Situations which are a hazard or show potential to
cause injury or illness will be investigated by a member
of the Executive.
4.2 All hazards will be reported in the Incidents and
Hazards Book, kept in the Foyer of the Church.
4.3 Corrective action will be recorded, and reported back
to the person who identified the risk hazard.
